The first capital letter is the severity of the bug. Severity ranges from A to E, with A bugs being the highest priority and E bugs being the lowest. A good rule of thumb is that Functionality bugs are B level, and visual bugs are D level.
The category goes into the square brackets. This is typically where you are in the game.
Categories include, but are not limited to: Tutorial, Management, Social, Store, ETC.
The title is a concise description of the bug. It should not be a complex description, since that will come later. It should instead be a quick, easy to read title that gets across the point of the bug.
The device is the device the bug was found on. You can find your device's name by going to your settings, About (phone or tablet), model number.
The OS is the OS of the device the bug was found on. It can be found in Settings; About;
Android version.
Staging for each test will be determined by the design team and will be different for each game.
Description is a full, detailed description of the bug. Reproduction steps will come later.
Times reproduced is how many times you reproduced the bug. Generally I would try to reproduce the bug a minimum of 3 times and a maximum of 7 times.
Reproduction. Here I would list the reproduction steps to replicate the bug.
Actual is the actual result of the reproduction steps.
Expected is the expected result of the debug steps.
Below is a real example of this QA process done by myself for the company PennyPop and their game Battle Camp:

Description: When already in game past the initial tutorial, the ability to connect the game with Facebook

1. Tap on "Social".
2. Tap on "Facebook Connect" Icon.
3. Tap on "Email or Phone" box.
4. Enter Facebook User ID.
5. Tap on Facebook Password.
6. Enter Facebook Password.
7. Tap on the "Log In" button
8. You are taken to a prompt that says "You have already authorized Battle Camp." and are given a choice to tap "Cancel" or "OK"
9. Tap on "OK"
10 Observe that nothing happens.
11 Observe that you have not gained 50 Stones.
12: Tap on "Social"
13. Observe that you are not connected to Facebook.
14. Tap the "X" to close the Facebook screen in the upper left corner
15. A message pops up with the subject head "Failed to connect to Facebook" with the message being "Failed to connect to Facebook. Try again later"
Actual: After logging into your Facebook account Battle Camp does not connect the game to your account
and does not give you 50 stones for this login connection.
Expected: After logging into your Facebook account Battle Camp connects the game to your account and
gives you 50 stones as a reward.
